Anyone ever appeal a health insurance denial of claim?

I had Medicare and a Medicare Supplement. I also had a Major Medical plan through an employer so that my wife could get covered who wasn’t on Medicare yet. I found that I needed major spine surgery and bot companies approved the surgery. I have been fighting both for over three years over around a $100,000.00 bill. Some bills were paid at first and then all of a sudden claims started getting reversed. Each company claimed they were secondary and the other company was my primary insurer. I spent hours on the phone for a year or so before filling a claim with my state Dept of Ins. Then I received a letter saying that they had no jurisdiction on Medicare claims and that Major Medical plan was issued in the home state of my employer and that I needed to contact that state. I contacted that state dept of ins. and found that even though is was a medical plan with a major insurer that it was company funded and therefore the state had no jurisdiction on that policy. Surgery was three years ago this month and still fighting who is going to pay. Both companies are constantly telling me they are in contact with the other company trying to get this settled.
